Referral and Prescription Policy
Rehabilitation services are available by physician referral. After you see your physician and obtain a prescription, call to schedule an initial evaluation. (Your doctor’s office may schedule this initial appointment.)
Initial Visit and Evaluation
During the initial evaluation, a history will be taken, an assessment will be made, and a treatment plan (goals, frequency and duration) will be formulated. Please wear loose fitting clothing appropriate to the body part being evaluated. Please arrive 10-15 minutes early to fill out the necessary paperwork, and plan on the initial evaluation lasting about one hour. Be sure to bring insurance cards, doctor’s prescription, and photo identification. Please call 24 hours in advance if you must cancel or reschedule your appointment.
Insurance and Scheduling
Due to insurance restrictions, your condition will be evaluated at the time of your initial evaluation. However, your treatment may not begin until insurance approval has been received.
Methodist Therapy accepts most insurance. As a hospital-based outpatient clinic, Methodist Therapy is currently exempt from Medicare caps on therapy services.
Make every effort to attend therapy sessions as scheduled to receive the most benefit from treatments. Canceling or missing appointments may decrease the effectiveness of the care received. Methodist Therapy reserves the right to discharge a patient from care if that person misses three or more scheduled appointments without notification of cancelled appointment.
